Providers wishing to participate with Meridian Health Plan must be enrolled with the Illinois Medical Assistance Program (MAP). Interested Providers, who are not already enrolled, must fill out an Agreement for Participation and a Provider Enrollment Application. Shown below are detailed instructions on how to enroll in the MAP, links to the appropriate paperwork, and directions on where to send the completed forms. Mid-Level Mental Health Professionals (LCSW, MSW, LCPC, LCP, CSW, LMSW, LMFT, PhD, Psy D, etc.) interested in participating with Meridian Health Plan should utilize the following forms for enrollment:
If you are already enrolled in the Medical Assistance Program, then you simply need to contact Meridian Health Plan at 1-866-606-3700 to obtain a contract for participation. If you are not enrolled in the program then you will be unable to contract with Meridian Health Plan. Sometimes if a Provider has not submitted a claim to the State for reimbursement within 18 months, that Provider’s file will reach a non-par status. The Provider must then call the Provider Participation Unit (PPU) at 1-217-782-0538 or write to the following location to verify the Provider’s address. Once this has been completed the PPU will reactivate the Provider’s file.
Should too much time elapse before contacting the PPU it could result in the provider becoming inactive with the MAP which means the Provider would have to re-enroll with the Medical Assistance Program prior to seeing a Meridian Health Plan member. |
