MERIDIAN HEALTH PLAN INTERNET PRIVACY STATEMENT
Collecting and using personal information
Meridian Health Plan's website is designed to provide important information to members and
health plan providers. Our site will gather and store non-personal information
from visitors automatically such as how many visited the website, what type of technology
they used, which pages they visited and how long they stayed on the website. This
information does not identify anyone personally and is used only to improve our
site and make it more useful for our members and visitors.
We will not collect personal information about you through our site unless you provide us
with that information voluntarily. By “personal information,” we mean
data that is unique to an individual, such as a name, address, Social Security number,
e-mail address or telephone number.
From time to time, we may request personal information from you at different locations on
our site (i.e. the Member Portal and “Live Chat”) in order to deliver requested
materials to you, answer your questions or deliver a product or service.
Upon your initial visit to the Member Portal, we will request that you provide us with your
Medicaid I.D. number, last 4 digits of your social security number, date of birth, first and
last name, email address and phone number. If you choose to provide it to us, we will
use this information to verify your identity, help you establish access to the Member Portal
and provide you with a user name and password to be used to access the Member Portal in the future.
If you choose to initiate a “Live Chat,” we will use the information typed by you
into the screen in order to assist in resolving any issues you are having. The information
typed into “Live Chat” is retained by us for limited period of time and may be used
by us for internal training purposes to improve the customer service provided to our members.
E-mail
Please note that your e-mail, like most, if not all, non-encrypted Internet e-mail communications,
may be accessed and viewed by other Internet users, without your knowledge and permission, while
in transit to us. For that reason, to protect your privacy, please do not use e-mail to communicate
information to us that you consider confidential. It is important that you understand that we cannot
guarantee the confidentiality of any e-mail while it is being sent through the Internet and use of the
Internet is solely at your own risk.
Requests for Protected Health Information
You have the right to request in writing information we have about you and to get copies of that information. MHP can deny your request for certain, limited reasons. If we deny your request, we will give you a written reason for the denial. We may charge a fee for copying your records. If you would like to request your protected health information (PHI), please contact Member Services at 866-606-3700. Member Services can provide you with a Request for PHI form and help you with completing the form.
